Everyone who participates in the testing session and answers an online health questionnaire will receive the “Know and Plan” $50 rebate. After that, earn an additional $50 “Next Steps” rebate by completing your choice of wellness activities from a variety of PEHP健康 activities and programs (more information available on the “Next Steps” Rebate Form (PDF)).

糖尿病患者（100美元）、戒烟患者（50美元）和Wee Care患者（50美元）也可以享受其他折扣。

您只能在每个计划年度或最后一次测试日期前60天注册一次健康犹他测试。 Log into your PEHP account, select "Wellness," then "Register for Testing" to figure out your testing session eligibility.
